Consent form for filming and photography
Over 16 year olds
Under the General Data Protection Regulation 2016 (“GDPR”) and the Data Protection Act 2018 (“The Act”)
by signing this form you hereby consent to the Coventry University Group using your personal data captured below,
and images of you taken in video recordings, interviews and/or photographs, taken/recorded today
at FabLab Coventry Eco Furniture Factory, Unit 4 Fargo Village, Far Gosford Street, Coventry, CV1 5BD for marketing and
publicity related purposes and used in other university publications and that they may be published on the university
website or other website or elsewhere and on social media. The personal data and video/photographs/interviews taken
will be securely held by the university for a period of 3 years.
You can stop the university using your images at any time, in which case they will not be used in future
publications but may continue to appear in publications already in circulation. To withdraw your consent
please contact studio.mea@coventry.ac.uk
You have the right to access information held about you, by making a data subject access request by contacting Data
Protection Officer at DSAR@coventry.ac.uk
You also have the right to apply for rectification of inaccurate data, erasure, and restriction of processing. You also have
the right to object to our processing and the right to withdraw consent.
